We may use and disclose your PHI for treatment, payment, and health care operations, such as scheduling, coordinating care with your dentist, billing insurance, and quality improvement. We may also disclose PHI when required by law or to public health authorities.

  • Inspect and obtain a copy of your health records.

Our responsibilities

We are required by law to maintain the privacy of your PHI, provide you with this Notice, follow the terms of the Notice currently in effect, and notify you following a breach of your unsecured PHI.
